Histopathologic differential diagnosis of neonatal cholestasis

OBJECTIVE: We aimed to evaluate the liver biopsy findings of neonatal patients with cholestasis and to determine the prevalence of biliary atresia and neonatal hepatitis.

METHODS: We retrospectively reviewed the liver biopsy findings of 153 newborn (0-4months) patients with neonatal cholestasis between 1992 and 2011 at Izmir Dr.Behcet Uz children’s Hospital. relationship of some parameters such as age, sex and findings of radiological investigations was assessed.

RESULTS: This series consisted of 93 (60.8%) boys and 60 (39.2%) girls with mean age of 73.8±30.5 days (10-120 days). Differential diagnosis was made with only one biopsy in 117 cases (76.5). Histopathologically 82 children (53.6%) were diagnosed as neonatal hepatitis and 71 children (46.4%) as diagnosed as biliary atresia. Kasai operation was performed in 23 children (32.4%) with biliary atresia and transplantation was performed in 11 children (15.4%). There was a relationship between the age of children and diagnosis (p=0.010).

CONCLUSION: In the present study, a realistic comprehensive picture of the liver biopsy findings of newborn babies with neonatal cholestasis was revealed. Then it was discussed the clues of differential diagnosis in these disorders.

Neonatal kolestazda histopatolojik ayırıcı tanı

AMAÇ: Bu çalışmada kolestaz saptanan yenidoğanlardaki karaciğer biyopsi bulgularının değerlendirilmesi ve biliyer atrezi ile neonatal hepatit prevalansının belirlenmesi amaçlanmıştır.

YÖNTEMLER: İzmir Dr. Behçet Uz Çocuk Hastanesinde 1992 ile 2011 yılları arasında neonatal kolestaz nedeniyle karaciğer biyopsisi yapılan 153 bebek (0-4 ay) retrospektif olarak incelenmiştir. Bebeklerin gün olarak yaşı, cinsiyeti ve radyolojik inceleme bulguları benzeri parametrelerin birbiriyle ilişkisi değerlendirilmiştir.

BULGULAR: Olguların 93’ü erkek (%60,8), 60’ı kız (%39,2) olup, ortalama yaş 73,8±30,5 gün (10-120 gün) bulunmuştur. Yüz on yedi olguda (%76,5) ayırıcı tanı tek biyopside yapılmıştır. Histopatolojik olarak 82 bebek (%53,6) neonatal hepatit, 71 bebek (%46,4) biliyer atreziyle uyumlu olarak değerlendirilmiştir. Kasai operasyonu biliyer atrezili 23 olguda (%32,4), karaciğer transplantasyonu 11 olguda (%15,4) uygulanmıştır. Olguların yaşı ile hastalık arasında istatistiksel anlamlı ilişki saptanmıştır (p=0,010).

SONUÇ: Bu çalışmada 0-4 aylık neonatal kolestazlı bebeklerin karaciğer biyopsi bulgularının kapsamlı profili çıkartılmıştır. Bu hastalıklarda ayırıcı tanı için ipuçları tartışılmıştır.
