Esophageal Foreign Bodies in Children: 2 Years Experience in a Country Hospital

INTRODUCTION: Foreign body (FB) ingestion is one of the most common cause of the emergency admission in childhood. In our hospital FB's of upper esophagus are removed with Magill forceps and the others with rigid esophagoscopy. The aim of this study is to present our patients treated with a diagnosis of esophagial foreign body in Muş Government Hospital for 2 years period.
METHODS: Patients diagnosed with esophageal foreign bodies who were admitted to Muş Government Hospital Pediatric Surgery Clinic between November 2013 and December 2015 were evaluated retrospectively. In our study 19 children with a diagnosis of esophageal foreign body were reviwed according to age,gender,clinical symptoms,radiological findings,localization in esophagus,and the type of foreign body.
RESULTS: There were 10 male and 9 female patients.The youngest was 1,5 and the oldest was 15 y.o. The most common location (52.5 %) of FB detected in esophagus was the first narrowing. We used Magill forceps for removing FB in 52.5 % of patients and rigid esophagoscope in 38.9%. The most common symptom was refusing of feeding and dysphagie.
DISCUSSION AND CONCLUSION: A detailed history is essential for foreign body ingestion. Pointed and sharp objects usually requires emergency intervention. Foreign bodies located in the upper part of esophagus can be removed easily by Magill forceps but rigid esophagoscopy remains a golden standard of removing objects
from the esophagus of pediatric patients.

Çocuklarda Özofagus Yabancı Cisimleri; Bir Taşra Hastanesinde 2 Yıllık Deneyim

GİRİŞ ve AMAÇ: Yabancı cisim (YC) yutma şikayeti çocukluk çağında en sık acil başvuru sebeplerinden birisidir. Hastanemizde üst özofagus yerleşimli YC’ler laringoskopi eşliğinde Magill pensi ile, diğer seviyedeki YC’ler ise rijid özofagoskop ile çıkarılmaktadır. Bu çalışmamızın amacı Muş Devlet Hastanesinde 2 yıllık sürede tedavi edilmiş özofagusta YC tanılı hastalarımızı sunmak ve tedavi metodlarımızı literatür ışığında incelemektir.
YÖNTEM ve GEREÇLER: Kasım 2013 ile Eylül 2015 tarihleri arasında özofagusta YC şüphesi ile Muş Devlet Hastanesi Çocuk Cerrahisi Kliniğinde yatırılan 19 hastanın dosyaları retrospektif olarak incelendi. Çalışmamızda özofagusta YC obstrüksiyonu şüphesi ile yatırılan toplam 19 hasta, cinsiyet, klinik semptomlar, radyolojik bulgular, özofagusta takıldığı yer ve yabancı cismin türüne göre incelenmiştir.
BULGULAR: On erkek ve 9 kız hastanın en küçüğü 1.5, en büyüğü 15 yaşındaydı. YC’lerin en sık birinci darlıkta (%52,5) takıldığı görüldü. Hastaların %52.5 sinde Magill pensi, %38.9 unda özofagoskop kullanılmıştır.En sık semptom beslenmeyi reddetme ve yutma güçlüğüdür.
TARTIŞMA ve SONUÇ: YC yuttuğu bilinen hastalarda detaylı bir anamnez şarttır. Sivri ve keskin cisimler genellikle acil girişim gerektirir. Üst özofagusta takılı kalmış YC'ler entübasyon gerekmeksizin Magill pensi ile kolayca çıkarılabilirler ancak daha alt seviyedeki YC lerin çıkarılmasında altın standart halen rijid özofagoskopidir.
