OBJECTIVE: The studies considering childhood renal disease epidemiology and clinical and histopathological findings are scarce in our country. This study aims to examine the clinical and histopathological profile of the renal biopsies evaluated at the Behcet Uz Children’s Hospital(BUCH)) and determine their status of correlation with clinical diagnosis.
METHODS: The documents of the 123 biopsies executed between 2009-2010 at the BUCH and Izmir Tepecik Training and Research Hospital Child Health Department and evaluated at the Dr. Behcet Uz CDH pathology department were assessed. All patients were considered all clinical characteristics.
RESULTS: The mean age was 9.4±4.0and 52% were females. Among the 170 biyopsy indications the leading one was Nefrotic proteinuria with 51,8 %. Among the 201 clinical diagnosis FSGS was the first in the rank with 25.9%. Mezengioproliferative glomerülonephritis, minimal lesion disease which were considered as normal under light microscope evaluation and the nondiagnosed spesimens which were only reported with their microscopic features were the three leading results of the histopathological evaluations. Clinicopathological correlation was achieved among 86.3% of the patients while this correlation was lost for tubulointersitisyel nephritis and chronic glomerulonephritis cases.
CONCLUSION: In this study the renal biopsy endications are consistent with the literature and nephrotic syndrome, proteinuria and hematuria are the leading endications. Among the nephrotik syndrome group mezengioproliferatif glomerülonefritis and ML disease is predominant while in the hematuria group the leading pathological diagnosis are HSV nefritis and IgA nephropathy. For achiving better clinicopathological correlation, electron microscope evaluation and genetic analysis is necessary.

AMAÇ: Çocukluk dönemi renal hastalıkları ile ilgili epidemiyolojik, klinik ve histopatolojik bilgiler ülkemizde az sayıda ve sınırlı araştırmaya dayanmaktadır. Bu çalışma Dr. Behçet Uz Çocuk hastalıkları Eğitim ve araştırma hastanesinde (BUCH) böbrek biopsilerinin klinik ve histopatolojik profilini incelemeyi ve klinik ön tanılarla korelasyonunu değerlendirmeyi amaçlamıştır.
YÖNTEMLER: : Retrospektif tanımlayıcı araştırma. İzmir Dr.BUÇH ve İzmir Tepecik Eğitim ve araştırma hastanesi çocuk servislerinde, 2009-2010 yıllarında yapılan ve Dr.BUÇH patoloji laboratuarında değerlendirilen 123 böbrek biyopsisinin arşiv bulguları üzerinden değerlendirildi. Tüm hastalarda yaş, cinsiyet, sistemik hastalık, ailede böbrek hastalığı öyküsü yanı sıra, biyopsi endikasyonları, klinik ön tanılar, hematüri ve proteinüri varlığı, laboratuar değerleri açısından ele alındı. Patolojik değerlendirmeye yeterli olarak kabul edilen 110 olgunun immünfluoresan (IF) ve ışık mikroskopik düzeyde incelemeleri esas alındı.
BULGULAR: : Olguların yaş ortalaması 9.4±4.0 olup %52’si kızdı.170 biyopsi endikasyonu içinde %51,8 ile Nefrotik proteinüri önde iken, 201 ön tanının dağılımı incelendiğinde FSGS % 25.9 ile ilk sırada yer almakta idi. Histopatolojik tanıların dağılımında mezengioproliferatif glomerülonefrit, ışık mikrokopik olarak olağan rapor edilen minimal lezyon hastalığı (MLH) ve mikroskopik tanımlama yapılıp, tanı verilmeyen Klinikopatolojik uyum %86.3 olguda sağlanırken, tubulointersitisyel nefrit ve kronik glomerülonefrit saptanan hastalarda bu uyum kaybolmakta idi.
SONUÇ: Çalışmamızda böbrek biyopsi endikasyonları literatürle uyumlu olup, nefrotik sendrom, proteinüri ve hematüri ön sıralarda yer almaktadır. Nefrotik sendrom grubunda bazı çalışmalarla uyumlu mezengioproliferatif glomerülonefrit ve MLH öne çıkarken, hematüri grubunda literatürle uyumlu biçimde en sıklıkla HSV nefriti ve Ig A nefropatisi tanıları yer almaktadır. Klinikopatolojik korelasyonu daha üst seviyelere çıkarmak için, rutin çalışmalara elektron mikroskopi ve genetik analizleri de eklemek yararlı olacaktır.
